Rinnai R75LS Tankless Heater - Hot 1 minute, Cold the next (Code 10)

Rinnai R75LS Tankless Heater - Hot 1 minute, Cold the next (Code 10)
Went out to my dad's again over Thanksgiving where he has a Rinnai R75LS, tankless water heater.
Over the past two years, the water heater doesn't seem to keep constant heat. They've looked at it in the past and just said it needs some cleaning and removed any moths that might of gotten in. Each time it's stated "fixed"....it seems to behave the same way.
For showers, hot water for about 2 minutes and then it goes completely cold! And remains that way for the rest of the shower time. Needless to say, my wife and kids don't like taking showers.
This time we experimented. After the 2 minutes of hot water, it went cold. So we turned it off for about a minute and then turned it back on. Hot water again!
At the same time it went cold, I heard some beeping so I checked it out.
The heater was flashing "code 10" and beeping.
As soon as it went off...hot water again.
So, what's up with this thing??
It is on Well water and it looks like he has a few "paper type" pre-filters and then into a tall cylinder "tank type" filter. I don't know the water quality of the well but could this contribute to the water tank not performing very well??

I have recently got an error code 10 on my r75ls. I checked - JustAnswer
That was one solution that worked for a customer who lived at a high altitude.

Check your vent
Code 10 is a blockage in your intake or exhaust. Check that a bird hasn't built a nest inside. If your dad lives where it's cold, make sure snow hasn't covered the vent.
