Ah, this article is absolutely not accurate. The author seems to be confusing direct vent fireplaces (which use outside air for combustion) and vent free systems that rely on air from inside the room.

With direct vent systems, both the combustion by products and the fresh air are circulated through a pipe that connects to the outside.
