What model is this motor

My compressor cooling fan motor is seized up on my small Nordyne central AC unit. I believe the bearings are rusted to the shaft. I cannot rotate the motor shaft using vise grip pliers.

Can anyone tell me what the model number of the motor is (photo of label below) and recommend an online AC parts vender I can order it from

Search for the fan motor by using your AC model number on a hvac parts site.

I ordered the motor on Wednesday, 3:30 pm and received it Thursday, 11:00 am. I should have ordered the cap also but I think I can get it locally.
