Crawl Space Encapsulation Questions

I have a small at grade crawlspace underneath an addition to the house, it has a small access door that connects into the finished basement. Currently it's just dirt floor and not vented. I'm wanting to encapsulate the space to help with air quality in the home and then to insulate the crawlspace walls in order to keep the addition warmer.

My main questions are:

1. What R value hard foam insulation should be used for the walls? I'm in Central Ohio.
2. Is the insulation attached directly to the block walls, or do you first take the encapsulating vapor barrier up the block walls?
3. What are some good sources for 10-20 mil vapor barrier?
