another broken exercise bicycle question

The bearings in my Johnson 2000 exercise bike fell apart and it ground to a halt. When contacting the store where I bought it 5 years ago I was told they would travel the hour to our small town to repair it for $250 or I could take it there to repair for $150 plus parts. All I needed were the parts and I asked for the repair number to order them. After 3 long distance phone calls I e-mailed the company to complain and then received instant help. The parts were shipped for a total of $30. But, in the meantime I had forgotten how they reassemble. Does anyone know how it goes together as there were parts left over when I finished (not a good thing) and there is rubbing.
The parts I received are: a fastener for the flywheel, 2 bearings and their housings, 2 washers and 2 more fasteners. Much grumbling and swearing went into getting it together the first time as the fasteners didn't want to go on unless seated exactly.

You don't mention grease for the bearings. Did you put any on them?
What parts were left over?
