Stair machine making loud 'thud' on each rotation

I have a stair machine which needs to be adjusted. The left pedal (not the pedal itself, but something involved with it) makes a loud thump every time the pedal rotates. I adjusted the pedal by unscrewing the nut shown below, repositioning the pedal and screwing the nut back in place but it made no difference. I don't actually know what needs to be adjusted or if the machine is poorly designed in the first place. The loud thudding is kind of annoying and I assume it's not good for the machine.

And we have no idea what model or make this is, how can we even begin to help?

Because maybe this is a common issue that someone who works with gears and nuts & bolts would know about. There's a sticker that says "Orbitrek - 137012". I don't see anything online for it. It's old.
