Glass Shade for my exhaust fan/light combo

I'm trying to find glass shade, on this bath exhaust fan/light fixture..
Can anybody ID the fixture/fan? or help me find the glass shade/cover?

the height is approx 5.3, width has little under 13" diameter, the whole in the glass cover the hang it is at least 1/2" in diameter

I do not want to replace the whole fan , just need a light shade. replacing fan would to go under operation to fix the fever..

Your text is a little hard to follow. I guess the long and short is you want to replace the broken glass.

I have a big lighting store near me that carries hundreds of replacement glass covers. I know they'll have it and I also know I'll pay a premium for it. I know the home improvement stores carry inexpensive lights fixtures where you could buy it for just the glass. An example of a 13" one is below. There are different sizes shown.

Thank you Pete. I tried that in HD, and others in Lowes and Wall Mart... but they didnt fit, the problem is that the hole in the glass is too small for the mounting bolt.

If someone can Id the model , I can just buy the same glass.

Another caveat is if I fix any glass, since the fan is covered by the light fixture, how will it suck/exhaust the vapor?

Sorry.... I can't ID the fixture. Possibly there may be an ID sticker somewhere on it that would help.
