Walkway sinking

My walkway along the side of the house is sinking to the point that when it rains the water seeps into the sunporch under the door on that side and under the cinderblock wall along the porch.
Can I repair the walk by breaking up the concrete in the walkway to roughly 2 inches down (into the 4 inch block) and then pouring new concrete over the existing block that remains and sloping the walk away from the porch the way it should go?
Should I waterproof along the wall to keep water from getting under the cinderblock wall also?

If the walk is sinking, removing it and making an adequate base to support it should be first. Then pour a new section at the appropriate level and grade.
