Question Building Forms for Ramp

I'm preparing to pour a 12'x12'x6" slab for an outdoor shed. Since I will be parking my lawn tractor inside, I'd like to pour a ramp as well.

The ramp will need to drop 4" to 5" to connect the top of the slab with grade.

What is the best way to build a ramp of this type? I'd like to pour it at the same time I'm doing the slab, but I don't know if I need a different slump factor or what. Any advice?

hi
just take a 2x8 and cut it in a slope to match your slab height. 0 to 6 inches hight
make sure you make a expansion joint where the ramp meet the slab
make it at least 36 inches long or + and the with to your door size
used same concrete mix as slab

The minimum thickness should br 4 inches.
