Putting in concrete walkway

I want to put in a concreat walkway along side my garage.Its over grown with weeds and is not even the same width. I have blocks and stepping stones in there right now. What kind and how much cement do I need. I was thinking of useing a bunch of 50LB bags of Quick Crete.and mixing in a 5 gallon bucket.What kind of prep work do I need to do and what kind of forms do I need.

Might consider doing a walkway with a Walk Maker:

I haven't used my recently, but at our previous house I did several walkways with it. It's a little easier than doing a solid slab and looks great. Plus you can do it a little at a time at your leisure (always good when doing DIY concrete work by the bag). I have the Country Stone pattern, but I see they now have quite a selection of types and patterns.

Surfed their site and I see they have instructional video on the site; pretty cool.
