Driveway 'concrete front lip'?

Guys,

I need some advice. I have an asphalt driveway that has a sunken front due to logging water. I've discovered at one point, my driveway did have the slope needed to fend off the water.

However, due to the water problem, the driveway condition is getting worse. I have a plan to make a 'concrete front lip', similar to driveways that has curbs on each side, which mine does not. I don't know what it's called, but I'll call it a concrete front lip.

I want to have it stick up roughly 1 1/2" above the road level, which will complete the existing slope of the driveway.

I wanted to know what type of cement I should use and how deep and thick I should make it. Also, should I put any rebar in it?

I'll back fill the sunken area, after I've compacted it, with ez streets cold asphalt and compact it.

You guys think this will work?
