Raised stone/morter foundation

Have an old house, with a crawl space, raised stone morter foundation. Everything looks good with the foundation very solid. To close off the raised foundation there was a concrete patch, with an extra concrete pad poured all the way around the house. problem is the patch is cracking and coming off, pad is starting to buckle a bit. My issue I don't know what to put in the cracking concrete to seal it back off. More of a cosmetic issue, but it needs to be addressed or moisture will start getting in. Any ideas would be great!

Pics would help but off the top of my head there is exterior caulking that could fill the cracks as well as mortar.
