How Big of a Hole Can I Put In My Foundation?

Over 30 years ago, we did a major addition to our home and the HVAC guys had to punch a hole through the foundation to get the duct work installed. Last week, we installed a heat pump and we were told that we now need a much bigger hole, 14"x14", through the foundation. The guys who installed the heat pump are unwilling to make the hole bigger. Is there a formula as to how big a hole there can be before it needs reinforcing? Or does it need reinforcing regardless? The original foundation was concrete, 4" thick.

So what is it now ?
Why does a heat pump require a 14" x 14 " hole ?
