using cat 5 cable for telephone

I would like to use a cat 5 cable for my telephone (modem to wall jack) what do I need to do

If you don't have Cat5 in the walls, there's not much reason to use Cat5 as a patch cable either - unless you just have nothing better to do with it.

one more question though Just to make sure that I am doing this right

this should work going to any phone jack am I correct

PS I just want to get it right to avoid and "smoke" further more I am in a jam and need a patch cable

As long as it matches the USOC standard on the other side of the jack, you'll be fine.
