Old Tile and Grout Causing Leaks?

Here is my situation:
- Water damaged ceiling below bathroom - moisture present
- Bathroom completely done in tile
- Home inspector said to replace wax seal to fix problem
Here is what I did:
- Removed toilet
- Inspected floor
- cut hole in ceiling to inspect drain pipe
Here is what I found:
- Badly stained tile under toilet (looks like mineral builup)
- Old and brittle gout
- Missing grout and caulk
- Slightly warped floor, causing cracks between tiles
- Only ceiling damage is below the toilet, not sink or shower
- Drain pipe appears to be fine
Here is what I need to know:
- Is it possible that the seal was the only culprit?
- Is it possible that the condition of the grout is contibuting?
- Should I put new grout over the old grout?
- Any other suggestions?

Yes. The wax seal was most likely the problem. I would remove the tile and grout in the affected area, inspect the subfloor and see if it needs replacement.
Adding new grout to old will prove a wasted effort since the bond will not take to the old dirty grout.
