Shower fixtures

I want to replace my 20 year old Delta single handle shower handle with a new Kohler model. I need to change the valve to accommodate the new fixture. Will it be necessary to tear the tile off the wall, make a larger opening to replace the valve and then do the necessary patching. The current opening is just smaller than the trim cover for the fixture.

No, do not tear out your tile wall.
Cut an access hole in the wall behind the faucet, centered on the studs to either side of the faucet.
You can either re-sheetrock it, or better yet for future access and repair, install cutoff valves on both water lines, and a cabinet door over the hole. Paint it or stain it to match your woodwork, even if it is in a closet.
