Old house basement door too narrow

Thanks for being here for a problem encountered today. Since I was here I thought I'd ask for my sis what to do about her door to the basement in an old 1880s house. The door is so narrow everyone, even skinny me, has to turn sideways to step onto the steps. The jamb can't be extended to the left as it is up against the kitchen wall, and the door is under the narrow stairs to upstairs. I'm thinking it would take structural work to the right and out into the entryway to make it bigger. What would that do to the steps up? Would the whole staircase have to be replaced?

How narrow is the door, in inches? How wide do you want it? How wide is the door casing, trim? A picture may be required to give suggestion.
