Stress crack...may have been so tiny you couldn't see it initially. Happens all the time with vinyl windows. We were supposed to inspect every window as it came off the truck and return the ones that had cracks (ha! Who had time to inspect 20 pallets of windows every week?)
As to the warranty, it depends on the maker and how long it has been since it was manufactured.
As to the repair...very few manufacturers will (w/o a lot of phone calls and letters) actually send someone to do the repair. Many will either want the unit returned and they will repair or replace it (if they cover it under warranty), or they will send you the parts and material to DIY.
Better companies like Andersen, Marvin, etc have regional service reps who will come inspect it, and decide if it's covered under warranty. (It better have been installed to spec, or almost no one would cover it after an inspection.) if its covered they would have a service company do the repair on-site, if possible.

Hi taser, I'm not famiriar with all of the northeast, but way up in Maine the consumer laws are among the toughest in the nation. Maybe your state as well. The last line of all warrenties must state something to the effect of "this warrenty gives you specific legal rights, you may have other rights which vary from state to state." Here is a link to Maine's list of laws, written so we can read them.Office of the Maine AG: Consumer Protection: Consumer Law Guide
