A door always closes when opened. How do I adjust the door to make it neither open nor close.

The door is not plumb and level.
Loosen hinges and slip hidden shims (I use piece of popsickle stick or old utility knife blades for this) behind bottom or top hinge to make door stay where you leave it.

Do you put the shims on the door side or the frame side? Would you recommend removing the door while putting the shims in?

Allen,
Put hidden shims under a hinge on the frame. Don't remove the door because you need to tighten the hinges back with the door in place to see if you need to add another shim.
Just slightly loosen the hinges on the frame, slip a shim under the bottom hinge, tighten the hinges back, and add/adjust as necessary, until the door stays exactly where you leave it. Easy.
