It's a vinyl threshold insert on an aluminum threshold.

This looks like the one but you would want to take some careful measurements to be sure.

Any Lowes, Home Depot, hardware store will have it.
Lot's of fun to try and install.
Clean out the groves with a small screw driver, vacuum it out, soaking the strip in warm water will make it more flexible.
Cut it to length.
Try to get both sides into the groves working it in from side to side.
Once both sides are in use a tapping block and a hammer or a rubber mallet with light blows to get it to seat all the way in.
