Raise external air vent?

I just mostly finished installing a new HRV system in my house, and while all went well, I'm not sure what to do about the exhaust vent. The problem is that when I punched through the rim joist for the vent (the unit is installed in my crawlspace), the hole came out such that if I just put a vent hood on it, it would be all of about 2" above the ground - obviously not good.

I tried looking for some sort of "offset" exhaust hood or the like, but haven't had much luck. What is the best way of raising the exhaust vent to provide some ground clearance?

The ducting is 6" round.

You will likely need to elbow and extend the duct using 50lb 6" pvc.
