Can someone tell me what vent this is?

There is a vent cover at the back of my house, but I am not sure what it is venting. I also have to replace it due to a crack. It looks like there are 4 screws holding it into the brick. Should it be fairly easy to remove? Thanks

Most often those are clothes dryer vents but it also could be from a bath exhaust fan if you have a bathroom in the basement.

Additionally it might even be a fresh air intake if there is only screen and no damper on it.

There is only a screen, no damper. It can't be venting the dryer because that is in a different location. I think it must be an air intake because I believe the bathroom fan vents in a different place.

A gas fireplace or gas water heater or similar sometimes requires that fresh air. At least some older installs were done that way.

An easy (but dirty if you have to go into the crawl space) way to tell is look on the other side of the foundation wall and see where it goes.
