refrigerator - heavy condensation and ice

On the inside top of my refrigerator (15 years old) there's heavy condensation and ice formation (in frig, not freezer). It drips and puddles onto the top shelf. The door seals are tight. Any ideas about how to diagnose/fix?

There are a few things you should check for.

1. Often when the freezer is not defrosting properly this will happen. Look for frost build-up on the evaporator cover. Refrigerator Not Cooling Repair Guide
2. Sometimes when the drain stops up in freezer the air ducts will stop up with ice. This can cause this problem. Look to see if the air ducts are clogged with ice if so remove the ice and unclog the drain. Refrigerator Leaking Repair Guide
3. If the refrigerator is in a high humidity environment this can happen even if the seals are sealing properly. If your refrigerator has a energy saver turn it off.
