New Panel for 200 AMP - Clarification on Inspector's notes

New Panel for 200 AMP - Clarification on Inspector's notes

I installed a new panel for 200 amp service. The city inspector came by and gave me a few items that needed to be rectified before it will pass, but I have a couple of questions on the inspector's notes.
1. "Identify ungrounded conductors in panel." Does this mean I have to write down (on the panel) which breakers aren't grounded? I live an an old house with some ungrounded wiring.
2. "Provide bonding for metal raceway in panel." I'm not sure what he meant by this.

Did the inspector have any code references next to the notes?
I can guess. Are any of the conductors connected to the circuit breakers colors white, gray or green? If so, he wants them reidentified. Is the panel schedule updated on the door, indicating what each circuit serves?
Does the incoming service enter the panel with metalic conduit. Is there a grounding bushing on the sleeve that the incoming service enters through?
Just some possibilities.
