Romex Clamp shorts out circuit

Last week installed a new circuit and used a 1/2 romex clamp on the box, went to trip on the breaker and it blew, after tracing my work it was the clamp that actually pinched the wire and shorted the hot to ground wire.
These clamps were bought at Lowes and in the middle of them have a little nipple that sticks out and penetrated the wire, thats what did it.

I think a bad product design, has anyone else ever experienced this?

They have always been like that. You just made the clamp part too tight on the wire.
