Service entrance panel
Does the main breaker always have to be at the top?
Can you turn the panel upside down and have the entrance cable lugs and main breaker on the bottom?
Can you turn the panel upside down and have the entrance cable lugs and main breaker on the bottom?

What you are bringing up is mostly a labeling issue, since electricity has no way of knowing if it is flowing up or down.
If the mfg has labeled the panel with a clear TOP then it must be installed that way. If not you can turn it upside down.
In addition, if the main breaker is oriented so that up is on and down is off, this must be kept. If the main breaker is installed sideways it does not matter.
Sometimes the "enclosure" (metal box) will be maked with a TOP label, but the internal "panel" (electrical guts) can be turned upside down. Part of the cover/door assembly may have to be adjusted as well.
