Usually licensed electricians have to fill these out, and most locales require this.

Is this a subpanel or a new meter service?

It's a new meter box... I called the inspection place and they said either myself or electrician could fill form out. I noticed to there's no specific place for a electricians license# where as on other forms I looked up do have form boxes specific for electricians only. I have a copy of the forms scanned if you'd like to see it in full, might help.

ok i can decode this for you

the 10 really mean 1 service
Cond (OH )mean overhead service drop cable size
lateral mean underground
Perm service just make a check mark depending on if you are overhead or underground cable
No.sets mean numbers of cables that will asked for paralled cable [ very rare for this but for now just wirte 1 ]

size mean the wire size in the meter box like 3/0 or #3 it depending on what you will install in there and make a note say CU or AL [ CU for copper AL for alum.]

Amps = size of main breaker either 100. 150 or 200

Service switch some case some area required a main switch before meter [ i doubt that but check out the POCO regulations for that ]

No.meters = numbers of electric meter will be in that place majorty of resdentail area useally is 1 but some case have 2 if dual rate meter for waterheater or specal useage

volts just type in 120/240 [ very common resdentail voltage unless poco say something diffrent like 208/120 that diffrent story ]

For the 30 = 3 just leave that part blank you dont need that
the last line the estamed falut current at line termails

this useally either 10 K or 22 K [ poco can tell you this ]

Hope this will help ya with the info

Merci, Marc

Thankyou!!! I didn't expect such a good answ so quick, so I'll leave the Serv.Sw blank for now and fault current I think would be 22 since live close to power station..etc versus living outin country...at least from what I've read. Sure appreciate!
