Location of GFI receptacle in bathroom

Hi,

I'm almost done wiring up our new bathroom. I've run #12 NM from the 40-amp attic subpanel to the bathroom for the dedicated receptacle circuit. Couple of questions:

1) Is there a minimum number of GFI receptacles required in the bathroom?
2) Is there a minimum distance a GFI receptacle must be from a shower or toilet?
3) What is the standard height above the floor for a receptacle next to the bathroom mirror above the sink?

If I put the receptacle on the left side of the mirror it'll be very close to the toilet; if I put it on the right side it'll be about 6" away from the glass shower wall.

Thanks as always!

The only requirement is that there must be at least one GFCI receptacle within 3' of the edge of the sink basin. It may not be located within the footprint of the tub/shower. Other than that anywhere you choose is fair game. Typical height is about 42" - 48" above the floor, but you can adjust that however is necessary to accommodate the cabinet, backsplash, mirror, etc.
