This thread is a compilation of numerous recall and safety threads throughout the years. Please reply with any new information you find and I will try to keep this summary up-to-date. Thanks to everyone who has pointed these out.

This otherwise high-quality book (Wiring a House) was found to have an incorrectly drawn diagram of a GFCI receptacle on page 188; however, the paragraph of text accompanying the diagram seems to be correct.

Aluminum Branch Circuit Wiring
Potentially found in homes built 1960s - 1970s, most dangerous pre-1972
Aluminum branch circuit wiring was used as a less expensive alternative to copper, however time has shown it to be a much higher fire risk due to severe corrosion at the connections. Not all types of aluminum wiring are considered hazardous; properly installed service entrances, feeders and large appliance circuits are acceptable. Please see this site for a detailed review.

Federal Pacific / Federal Pioneer / FPE Stab-lok Panels and Breakers
Various models dating back to the 1960s
Despite numerous investigations, there has never been an official recall of these products, but they are widely known in the electrical industry to be defective and a fire hazard. Please see this site for a lot of information on the issue.

Note: It would be better to replace these types of covers with modern Tamper-Resistant (TR) receptacles as they are required for most areas following the NEC 2008 edition.

Cooper Crouse-Hinds non-metallic, polycarbonate ceiling boxes Recalled

This recall involves five models of Cooper Crouse-Hinds non-metallic, polycarbonate ceiling boxes with manufacturer or part numbers TP16002, TP16022, TP16023, TP16122 and TP16307. The black, round ceiling boxes are 1.5 to 2.25-inches deep and measure between 3.5 and 4-inches in diameter. These devices are used in the installation of ceiling lighting. “CROUSE-HINDS” and “14.8 CU. IN” or “20.8 CU. IN.” appear inside the box. The part number is on the product’s packaging.
Incidents/Injuries

Cooper Crouse-Hinds is aware of one ceiling box cracking. No injuries have been reported.

Schneider Electric Recalls APC Surge Protectors

Description

This recall involves APC 7 and 8 series SurgeArrest surge protectors manufactured before 2003. The model and serial numbers are located on a label on the bottom of the surge protector. The two numbers that follow the first letter or letters in the serial number sequence indicate the year of manufacture. The unit is included in the recall if the numbers are 93, 94, 95, 96, 97, 98, 99, 00, 01 or 02. APC and the words Personal, Professional, Performance or Network are printed on the surge protectors. The following model numbers are included in this recall.

Incidents/Injuries

The firm has received 700 reports of the surge protectors overheating and melting and 55 claims of property damage from smoke and fire, including $916,000 in fire damage to a home and $750,000 in fire damage to a medical facility. There are 13 reports of injuries, including smoke inhalation and contact burns from touching the overheated surge protectors.

I don't think so, Kidde owns them now.

My FireX 12000 smoke/CO detector started chirping and changing the battery did not help. I eventually blew it out with compressed air and now it seems to be okay; but it is 7 years old and apparently that is about the life of a smoke/CO detector.
Are you aware the 12000 series Firex alarms were recalled? Yours may have been produced before the recall dates, but I'd check them to be sure.

On May 15, 2008, Maple Chase Company, manufacturer of Firex � branded safety products, announced a voluntary recall in conjunction with the U.S. Consumer Product Safety Commission (CPSC) of its 10000 series carbon monoxide (CO) alarms and 12000 series combination smoke/CO alarms, produced between June 1, 2007 and February 1, 2008. This corrective action is being taken because Maple Chase has determined that the affected models will enter into an audible "double chirp" fault in the presence of carbon monoxide. However, this audible fault also may occur when there are no elevated levels of CO present. The owner's manual for the affected models instructs the consumer to take the unit out of service if the fault continues after pressing the reset button. This results in the potential for the consumer to take the affected model out of service at a time when there may be a CO emergency. As a result, you are advised NOT to take your affected models out of service should they enter into the "double chirp" fault mode.

Firex addresses this under FAQ.

Reset the alarm.

Most current model smoke alarms have a processor that retains certain error conditions, such as a weak battery. The error condition should reset when the battery is replaced but sometimes it does not and must be manually reset to clear the error.

To reset an AC unit with a battery backup:

Turn off the power to the smoke alarm at the circuit breaker. Remove the smoke alarm from the mounting bracket and disconnect the power. Remove the battery. Press and hold the test button for at least 15 seconds. The unit may chirp or alarm for a few seconds before going silent. Reconnect the power and reinstall the battery. The unit will chirp once when the power is restored and should not chirp afterwards."

More Than 1 Million KIDDE Smoke Alarms Recalled Due to Alarm Failure | NBC New York

Three Kidde smoke alarm models were recalled Thursday due to the possibility of alarm failure following a power outage, according to the Consumer Product Safety Commission.

The recall applies to about 1.2 million residential smoke alarms in the US and 112,000 in Canada. No injuries or incidents have been reported.

The models are the Kidde i12010S with manufacture dates between December 18, 2013 and May 13, 2014; combination smoke/CO alarm il2010SCO with manufacture dates between December 30, 2013 and May 13, 2014; and combination smoke/CO alarm model KN-COSM-IBA with manufacture dates between October 22, 2013 and May 13, 2014.

The smoke alarms are hard-wired into a home's electric power. Two of the models come with sealed 10-year batteries inside.

The alarms are round, white, and 5 to 6 inches in diameter, with "Always On" engraved on the front of models with sealed 10-year batteries inside. Amazon.com, HomeDepot.com and shopkidde.com are a few of the retailers who sold the alarms, from January 2014 through July 2014 for between $30 and $50.

Consumers are advised to keep using the recalled alarms until they install replacement alarms. Consumers can call Kidde at (844) 553-9011 from 8 a.m. to 5 p.m. ET Monday through Friday to request a replacement or go online at www.kidde.com and click on "Recalls" for more information.

Recalls are a constant issue, here is one for a shop light sold exclusively by Home Depot. I would appreciate it if a Moderator would add this to the recall sticky.

The 120-volt LED bulbs, sold as 6- 8- and 9-watt bulbs (equivalent to 40 or 50 watts), were marketed under the brand names Definity, EcoSmart, Sylvania and Westinghouse. The model numbers A19, G25 and R20/PAR20 are found on the packaging and on the light-colored circular neck above the base of the bulb where the date code is also printed.

1/2 million bulbs included in recall. See link for date codes affected under recall.

This recall involved the LED lamps by Cree to replace 4 foot T8 fluorescent lamps. Over 700,000 of them being recalled in the U.S. and a few more in Canada.

Kidde fire extinguisher recall

No question, just a suggestion to check it out if you might be affected, but I happened to see an article on the local online news this afternoon announcing a recall on certain Kidde fire extinguishers with plastic valves, the type commonly sold at places like Sears, big boxes, etc. for home, shop, and vehicle use. The Kidde site has the details, along with the affected model numbers for the various names they were sold under.
