Wall switch to control electrical outlet for a dishwasher... Required?

Is a wall switch to power an outlet that the dishwasher is plugged into required? I don't really see the need for one. If you are working on the electricity, or if you need to disconnect the dishwasher, then the safer thing to do is turn off the breaker.

Do you guys put a switch in for a dishwasher? Where? On the wall? In the cabinet?

No, no switch or disconnect for a dishwasher.

With a receptacle, no disconnect needed.

Hardwired, we put in a switch. The local inspectors require it. We can put it on the wall or under the cabinet or sink.

Check with your AHJ.

Generally you do need a disconnect, but the plug can be removed and qualifies as the disconnect. That being said, that are many, many thousands of dishwashers out there that were originally hardwired that do not have any type of disconnect.

If the DW is hardwired either a switch or a breaker lock is required to serve as the disconnect.
