Help repurpose exhaust fan

I've got this fan I want to use but I'm not exactly sure how to wire in a power cable. According to the sticker I'll need a capacitor inline. Would someone mind showing me a simple layout on how to attach a three prong power cord to the wires shown on this label?

I'm a little confused by the INPUT:155/145 label, but it clearly says 120V 60 HZ, so white to white, black to Blue and violet. Cap goes between orange and red. Green wire from the cord attaches to metal frame of the motor.

Since it's a small motor, you can probably get by with a cap like this one: http://www.amazon.com/CBB61-Motor-Ca...HZ2ZGYRRZNYJQP

black to Blue and violet
I think CT meant blue or violet. It is a two speed fan. One is high speed the other low speed.
