motor start capacitor

I have a dental chair and the motor to move the back down isn't working. I think it's a bad capacitor. The capacitor is aerovox, 330v, 41-49MFD. Other numbers on it are 10-2002 ( I think this is manuf date), "C" (not sure what that is), PSA4R3341N (model #?) and lot PS009168.

I looked on Grainger and don't see any cylinder/round motor start capacitors that are close.
