I have seen quite a few ground boxes, but they have all be similar to Ray's picture. The top of the box is flush(ish) with the surface. I have found boxes that have been buried by mulch/rock...stupid landscapers.

I also confirmed that the upstairs light switch that is part of this circuit does have both the hot (black) and white and ground all connected. So what I did was to wire the outlet so that it is hot all the time
This is just a FYI..... the receptacle was switched with the outside light switch because at one time there was a low voltage transformer plugged in there feeding that now unused low voltage wiring,
