Need some expertise with a breaker box

When I check each leg of my water heater breaker they each read 120 respectively. When I measure them together they only read 26. I also get the same reading of 26 together at the main breaker in the sub panel. And that's as far back to the meter box as I've gone so far. Any help is greatly appreciated.

One of the hot legs is dead. Check the breaker feeding the sub panel. Turn it off and back on to make sure it is reset on both sides.

Do other large 240 volt appliances work properly? You may need to call the power company.

If the water heater is being fed from the sub panel and that is the breaker you tested as you reported did you check the breaker feeding the sub panel?

New dual AFCI/GFCI 20 amp breaker sparked

I have a subpanel in my garage and wanted to replace a regular 20 amp Siemens breaker with a dual function AFCI/GFCI 20 amp Siemens breaker. I took the old one out, and put the new one in. I ran the white neutral from the romex cable to the breaker, the black power to the breaker, then the white pigtail to the neutral bar. I flipped on the power at the main panel, then flipped the new 20 amp dual function breaker and it sparked and tripped. I powered everything down, installed the old one and the old one still works. What did I do wrong? Also, what is the hard, white substance on the old breaker? See pics below. 3 way switches..only one works at a time
