How do I get rid of condensation that builds up inside windows?

I get condensation building up between my windows. Could this be a indication of a insulation problem and how do I stop the condensation from forming?

The seal between the two panes of glass is broke. This allowed air to get inbetween them and condensate. There is absolutely no way for you to clean inbetween the two panes. The only way to resolve this is to either replace the thermal pane glass or replace the window.
