fireplace box ht. on hardwood floor?

Hi,

I'm wondering how high the fireplace box opening needs to be off the hardwood floor.

Are there certain height requirements according to various flooring material?

If I add a stone or metal hearth that extends into the room can box opening be lower to the floor?

A friend of mine purchased a used free standing (not set in wall) brass gas fire place with broken pieces and cracks in the liner. The liner is approx. 1 and 1/2 inches thick and has the appearance of grey colored concrete. Can the liner be repaired by a non-pro and at what cost? Do the few cracks need to be sealed? I want to convert an existing firplace to gas. Can I do this myself? I know that I would need a permit. My plan is to bring in a 1 inch gas line (How high off the floor of the fireplace should I bring in the line?) I am interested in doing the conversion as cheaply as possible.
