Ugly fireplace and hearth

We had our home built 12 years ago and selected a stone surround gas fireplace. We’ve had issues with the stone surround from Day 1. The mortar crumbled and stone started coming off within a couple of months after moving in. The builder fixed this. After having hardwood floors installed years later, the flooring company pointed out that the hearth was improperly built. As you can see from the attached pictures, we can lift the hearth because the mortar has crumbled. There are no cinder blocks or real support for the hearth stones. Shouldn’t there be some better support underneath the hearth? Should we just try to put in cinder blocks for support and some new mortar? Part of me just wants to hire someone to redo the whole thing.
