Terrible noise

Terrible noise

Has anyone ever heard of a center piece of floorboard going bad? Like I stated before I cut a piece of the floor out and glued and screwed back down where the noise was comming from. I evev drove 3" 3/8th lag bolts with an electric impact. The noise still persists. I ran out of time and had the room recarpeted. The creaking noise is terrible. If I was to consider taking the carpet back up and replacing the plywood what will i be looking at as far as headaches? Thanks for all your info.
