Plywood on concrete

Hi all I have a garden unit(half below grade) condo with hardwood floors. I have concrete then plywood then black paper then hardwood (oak). Is this standard installing technique. It first warped at seams. I had him replace some and sand other spots. The new stuff warped again so he sanded that and now seems ok. Moisture ? Thanks

yea its moisture i.m.o. they should have installed a vapor barrier under the plywood, not a just a vapor retarder (black felt paper), especially if your floors are below grade.
