The hinge pin part of the hinge will always stick out (not be flush) if you want it to open outwardly. Curious question.....why must the hinges be flush, and why do you want to use a piano hinge??

The hinge will not be flush in order to open 180.
Open the hinge all the way until it forms a 90 degree angle. Set the hinge on either the door or jamb. Predrill the holes and then screw in. Remove it and place it on the other side to do the same thing. I recommend putting in 3 or 4 screws on each side at first, then hang the door and finish the rest with it hung.
