Grounding breaker box and Meter

Grounding breaker box and Meter
We are remodeling an older home, and I am upgrading the wiring from 60 amp to 125 amp. I was told by the local inspector that I needed to put in two grounding rods. My first question is: Do I run a ground wire from the meter to one rod, and from the box to the other?
My other problem is with the service line on the incoming side of the meter. I was told to have four wires going into the meter, but there are only three wires coming to the house. Why would I need four wires going in? The service cable from the meter to the box has four wires also. The old service cable had only three wires. Is this going to cause a problem when the power company comes out to reconnect?
Thanks in advance for any advice.
