So is this a problem with your remote or with your tv? If you have controls that are actually on the tv, try using the manual controls instead of the remote.

This happened to me recently, too. Not sure if this applies to you, but here's what i did. My cable provider's remote controls allow us to "synch" the remote to the tv, preventing the need for 2 remotes (1 tv, 1 cable box). When the tv volume jacked itself to maximum not even the physical buttons on the tv could lower the volume. I re-sync'ed the cable remote to the tv, and that solved the problem.

Have you tried anything outside the box, like maybe trying to plug in a set of old computer speakers or maybe a sound bar? They would have their own volume control that you could adjust.

Look at page 20 of the manual above -
Most likely it's a combination of AVL + Sound-Mode

AVL "Automatic Volume Level" which makes quiet sounds loud, and loud sounds quiet.
Sound-Mode determines how much "spread" the sound has.

When you get an odd combination, you get too quiet for quiet, and too loud for loud.

Worst case, try a "factory resest" from the remote.

You MIGHT also check for a firmware update for the TV

Usually, that
