crawlspace moisture problem/excess house moisture

help one year ago our hot water heater leaked and caused excess window condensation this is how we found the leak and major mold problems in the house that was fixed. This year about 1 month ago the windows were getting wet again ckd. crawl and all the sand was wet, we were told in past to add 6mil moisture barrier, we put in a dehumidifier & space heater & opened heat duct into crawl it has dried up the sand, no water in dehumidifier(closed the 2 vents to dry up sand).Can we now just install plastic & be ok do we open up vents or do we insulate also & close vents use heater still & dehumidifier or what help!! we have had dehumidifier going constantly in the house lots of water being drawn out All hard wood floors thruout house built in 1960.
please help we need to do something asap thanks

I dont know for sure to say move the sand and put a 6 mil poly down and the sand back on top. Or put the 6 mil poly on top of the sand. On top or under, Over lap it 2 ft tape all seams up the wall about a ft. close all outside vents never open them again.Insulation on the walls.
