New ceiling lighting - need access to attic?

Our bedroom needs light...badly. At the moment we don't have any light besides a couple of lamps. Would I absolutely need access to the attic to put in a new ceiling light fixture (either light or fan)? I can get to the attic, but the crawl space is very tiny and lots of loose insulation. Any thoughts on this?

You need to be able to get a wire from the wall switch to the fixture location. If you can do that without going into the attic then I guess you don't need to go in the attic.
