Loose ceiling fan box?

Loose ceiling fan box?

I am looking at a ceiling fan that wobbles. My first thought was that it just needed to be balanced; however, while looking at it I discovered that the entire box is loose. What is strange is that there is a 2"-3" by 6"-12" metal plate with a hole in it for the wires that is between the electrical box and the drywall. Is this some sort of a old retrofit job to add the fan originally? Do I need to take out the plate and the existing box and put in a new retrofit brace?

Looks like a 4" square or a 4" round blank with a KO and a half of an NM connector in it.

Remove the fan and fan bracket and give us another picture.
