2 wire ballast to 5 wire-- HELP!

2 wire ballast to 5 wire-- HELP!
I have an old 14" fluorescent light that recently started to hum loudly. I found the replacement ballast, when I opened up the light I was shocked to see only 2 wires coming from the old ballast, both black! Does anyone know how I can hook up the new one?

Your fixture uses a socket with the starter. That would need to be replaced with a regular socket.
