Is this box rated for ceiling fans?
House built in 05, textured over most of writing, I can make out UL stamp, Bowers, but not if fan rated, looks like posts instead of ears, but is plastic

Welcome to the forums.
It looks like a generic plastic "nail on" box. It would not be fan rated.
Go back up there.... see if the box moves easily. Take the flat bar off and push up.
I'm pretty sure it will move easily and it will need to be replaced.
