Fixing noisy fan question

I've got a noisy fan. It's one in a pair. The other one makes no noise at all. Any suggestions as to how to fix this? Here's a video of the sound:

There are two types twist one and just regular wire nut. I have regular wire but but when joining two wires it is still loose and comes off easily. Is the wire nut suppose to do that or twist and not come lose?

I am trying to find a fluorescent fixture 2ft long that uses T12 bulbs. No luck searching the net. anyone have an idea where to look?
