Ceiling Fan Bracket \ Housing Loose

Good evening and thank you for taking the time to read the thread.
I've been tightening all of my ceiling fan set screws, but the worst one is loose at the fixture, specifically the retaining bracket where the sleeve at the top of the stem inserts, and the bracket screws into the ceiling.

It looks like whoever installed it did something wrong, and put a nut and spacer on one of the screws. Not sure what to do, but I did pick the fan up to take the load off and tried tightening both the screws and the nut. The nut just keeps spinning, and the screw is totally tight, can't tighten it any more. The bracket itself has a small amount of play in it and the fan wobbles like crazy.

First off is it a fan rated box? I assume it is.
Without the fan running (or detached) is the box loose on the ceiling?
That amount of wobble does not seem to be because of a loose box but because of the long length of the spindle and not being properly balanced. Is the wobble the same at all speeds?
Using clay try balancing the blades.

Can't tell for sure from the video but that looks like a black plastic electrical box.
If that is correct.... doubtful it's a fan duty box.

We could use a very close up picture of the box and mount.......... How to insert pictures.

Thanks, I will get close ups of both. The bracket seems to be made of metal, I tapped on it with my screw driver before I made the video. I'll get up there after dinner for pics.

I got the best photos I could, hopefully they are good enough. Tag says it's a fan bracket.

That tag is from the fan telling you must use a fan rated electrical box. It does not confirm that your box is fan rated.
However, can you answer my previous questions?

In the video.... the bracket is clearly loose where it connects to the box.
The blades look to be slightly out of balance too.
That type of fan hangs on a ball so off balance issues are magnified.

Not sure what you have going on there. You have two different fasteners.
One looks like a hex head machine bolt and the other looks like a machine thread screw with a stripped head. Your box may be fan rated but those fasteners should be the same and correct for the box used. My recommendation is to remove them to see what type/brand of box that is.
