Ice dams

My house is a 50-year old cape. In the fall I had insulation put in - fiberglas batting, with reflectex on the slopes - proper vents, a ridge vent and soffit vents - the little round plug type. Also the entire ceiling on the second floor was replaced with new drywall. Now that winter is here I still have a serious ice dam problem on both sides of the roof - one facing north, one facing south. Although the upstairs is much more snug and I'm using less oil, I'm very concerned that this seemed to have no affect on the ice dam problem. Any ideas?
